Sustainable Membership
This is the standard primary care membership.
One-time new patient enrollment fee: $100
Monthly Fee:
Age 26 and up: $90/month
Age 25 and under: $75/month
Save 5% when you sign up for 12 months in advance.
Group discount for families, friends, or any members paying together:
5% off for 2-3 members
10% off for 4 or more members.

Accessible Membership
If you are unable to afford the fee for a sustainable membership, a reduced cost membership may be available as resources allow. You’ll get the same care from us regardless of which membership tier you join. There is no income requirement for the accessible membership; we trust our patients to assess their own needs.
One-time new patient enrollment fee: $25
Monthly fee:
Age 26 and up: $50/month
Age 25 and under: $35/month

Equitable Membership
For those able to pitch in, joining with a higher cost Equitable Membership helps us to cover the cost of care for those with fewer resources.

Monthly Fee:
Age 26 and up: $120/month
Age 25 and under: $85/month

Long-Acting Contraceptive Placement and Removal
We charge a flat fee of $150 for non-members seeking placement or removal of Nexplanon implants or IUDs. For placements, this does not include the cost of the device itself. We can help you to get the cost of the device covered by insurance if you have it, or to apply for patient assistance from the manufacturer if you don’t.

One Time Urgent Visit for Member's Loved Ones
We are NOT an urgent care. However if we have availability we may consider seeing our patient’s loved ones for one-time urgent visits—for example family members who get sick while visiting from out of the state or out of the country. These visits will typically cost $150.
