What patients do you see?

We see patients of all ages and genders. We explicitly welcome people who identify with groups that are often underserved by mainstream medicine such as LGBTQ+ patients (including those seeking gender affirming care), Spanish speakers, autistic patients who challenge the medical model of autism, patients seeking treatment for opioid addiction, and patients who wish to take a size-neutral approach to their health. This FAQ answer is probably as good a place as any to acknowledge medicine’s racism problem; Dr. Mirer is just another white lady but she will do her best.

What is Direct Primary Care?

Direct Primary Care is a membership-based model for health care. Patients pay a monthly fee, and in return get comprehensive primary care with a personal relationship to their doctor. We cut out insurance company middlemen, which allows us to keep our overhead low and our costs affordable. The practice is small, with only about 15-25% the number of patients at a traditional insurance-based practice. That means it is easy to get quick appointments, and you don’t have to delay your care or waste money on avoidable urgent care or emergency department visits. Our staff is also small (that’s part of our cost-saving concept), which means you know the team and you don’t have to fight through an army of phone operators to talk to your doctor. Is this for uninsured patients?

Everyone is welcome at Presence, regardless of whether you have health insurance or what kind of insurance you have. Your membership fees cover the care that we provide. We do our best to help uninsured patients find the most affordable options for other health expenses such as medications, testing, or referrals. Insured patients may choose to use Medicare, Medicaid, or private insurance to pay for these expenses. We do not bill insurance for membership fees, and we legally cannot bill Medicare and Medicaid for that.

Is this health insurance?

No. We advise that our patients obtain health insurance to cover types of medical care beyond what we provide—for example hospitalizations, surgeries, emergency department visits, or specialty care.

Is this concierge medicine?

Not exactly. Concierge practices typically charge a fee to the patient’s insurance company and then charge an additional fee to the patient in exchange for increased access to their doctor. What we do have in common with concierge-style practices is small size and improved communication. But when you think of Dr. Mirer, we don’t recommend you picture a guy in a hat that makes restaurant reservations for wealthy hotel guests. That is not her vibe.

When can I join?

We are now accepting new patient enrollments. Until our clinic space opens in September 2024, Dr. Mirer is seeing patients for video visits and home visits. We are at 2500 W. Layton Ave, Suite 260, in Milwaukee’s Castle Manor neighborhood on the South Side.
